Subject:          Budget Amendment for HSW at Risk Performance Fee

Appropriate $25,000 from Fund Balance - General Fund.  Funds will be used to pay Health & Sport Works, Inc the at-risk performance fee per Article 4 b of the agreement as a result of FY 2016 operations.   


ACTION RECOMMENDED: 

Approve Budget Amendment

 
 
FINANCIAL IMPLICATIONS:

$25,000 appropriated from General Fund - Fund Balance.

 

The HFFA metrics developed in Schedule B of the contract indicate weighted basis for evaluation.  Scoring is weighted as follows:

 

Goal 1:  25%, not met

Goal 2:  25%, met

Goal 3:  25%, not met

Goal 4:  15%, met

Goal 5:  10%, met

 

Details regarding each goal are attached. 

 

The Contract indicates an annual performance fee not to exceed $50,000; with 50% of metrics met, this request allocates $25,000 (50% of $50,000) to comply with FY 2016 contract.
